How NJ Medicaid Transportation Works
NJ FamilyCare includes NEMT as a covered benefit for eligible members. The transportation program is managed through Modivcare (formerly LogistiCare), the state's NEMT broker contracted by the New Jersey Division of Medical Assistance and Health Services (DMAHS).
Modivcare coordinates transportation for NJ FamilyCare members by dispatching approved NEMT providers. Members do not contact NEMT providers directly for Medicaid-covered rides they contact Modivcare.
Who Qualifies for NJ Medicaid Transportation?
NJ FamilyCare transportation is available to members who:
•Are enrolled in NJ FamilyCare
•Have an appointment with a NJ Medicaid-covered provider
•Have no other available means of transportation
•Are traveling to a covered medical service
Note: transportation is specifically for medical appointments. Non-medical trips are not covered under the NEMT benefit.
How to Schedule a Ride Through NJ Medicaid
1.Call Modivcare's member line the number can be found on the NJ FamilyCare website or your Medicaid ID card documentation.
2.Have your NJ FamilyCare ID number, appointment date and time, provider name and address, and mobility needs ready.
3.Request at least 3 business days before your appointment for standard rides.
4.For recurring appointments, request standing order scheduling so you do not need to call each time.
5.Confirm your round trip requirements and expected return time.
Types of NJ Medicaid Transportation Available
Modivcare dispatches different vehicle types based on member needs:
•Wheelchair-accessible vans (WAV) for members who use wheelchairs.
•Ambulatory transport for members who can walk with assistance.
•Taxi and rideshare coordination for members who are ambulatory and live in areas with taxi or rideshare availability.
•Mileage reimbursement for members who have a family member or friend who can drive them. The volunteer driver receives mileage reimbursement from Modivcare.
NJ Medicaid Transportation for Dialysis and Chemotherapy
Dialysis and chemotherapy patients are among the highest-volume users of NJ Medicaid transportation, attending multiple sessions per week. For these patients:
•Contact Modivcare to set up a standing order for your entire treatment schedule.
•Provide the dialysis center or oncology clinic's schedule directly, including any shift changes.
•Keep Modivcare updated immediately if your schedule changes.
What If Your NJ Medicaid Transportation Is Denied or Late?
•Denied request ask Modivcare for the specific reason. Appeal through the NJ FamilyCare appeals process. Contact your managed care plan if you are enrolled in one.
•Late pickup contact Modivcare dispatch immediately. If the ride is significantly late, ask the medical facility to note the transport issue in your file.
•No-show file a complaint with Modivcare and document the incident. Repeated no-shows by the provider can be reported to DMAHS.
Private NEMT as a Supplement to NJ Medicaid
